If you have previously been referred under a Mental Health Care Plan (from a health practitioner) and you are aware that you have remaining sessions on this plan, you can simply book a consultation with Amber through our online booking system.

If you are unsure, do not have a MHCP or have not been to your General Practitioner in the last 12 months for your mental health, it is advised that you book a GP consultation to complete a MHCP referral.

Transferring your records to Amber will help to support your ongoing therapy. Your records and client file is your property, and you have the right to request this information to be transferred from a previous practice – please make contact with a previous clinic you have attended and request a copy of your file or for a copy to be sent directly and securely to Amber Vonhoff Psychology.

With your written consent, Amber Vonhoff Psychology can request the file on your behalf to be used as part of your ongoing therapy at Amber Vonhoff Psychology. This right is part of the Commonwealth Privacy Act, or under a Freedom of Information (FOI) request.
